Highlighting the same, Tiffany shares a success story wherein the company helped a hospital system relieve its case managers from exhausting work. The case managers had to undertake additional patient discharge tasks as well, which proved to be overwhelming. PhoenixMed helped develop a post-acute care resource center to centralize and handle all the acute care discharge related-tasks. Hospital staff could directly engage with the resource center rather than going through the case management department. This helped the case managers save time on discharge planning so they could focus more on adding value to solving complex cases. Even during the pandemic, when attrition was an issue, the hospital was able to function well with fewer staff.
